AFA '08 Tour Application Page
Wow, you’re going to have a blast with us on this tour! So, without further ado, let’s get down to business. We need you to supply us with the following details:
- Full name (as printed on your I.C.)
- MMU Student ID (for membership verification)
- I.C. number
- Contact number
- Full e-mail address
- Passport number (put down “group” if you require a group passport)

Payment Details
1. Please make your payments before 12th November 2008. Applicants with late payments will have their slot forfeited and given to applicants on our waiting list.
2. You can make your payments through CIMB using the following details:
CIMB Account No: 1255-0004344-52-3
Name: FAZRI NUHA BIN YUSOF and IVAN MOA
3. If you have made your payment, please attach a screenshot of the transaction OR a scan of the transaction receipt together with your application email. This will be your proof of payment.
4. If you made your payment after your initial application, please include your name in the email together with the screenshot/scan.
5. We will also set up a booth in Hostel Block 2, MMU Cyberjaya campus come the next semester to accept payments in person.

- Application by means other than email (phone, IRC, forum etc.) must be confirmed before 31st October 2008 or it shall be forfeited.
